to inform you or your authorised representative of how we manage
your personal, sensitive and health information and maintain its

sensitive and health information highly secure, we also believe that
you or your authorised representative have the right to be informed
about how we deal with that information so, if on reading this

the Safe Return Home service you or your authorised representative

from you directly or from your representative.
Your representative may be one of the following:

A guardian

An attorney under an enduring power of
attorney

of attorney (medical treatment) in accordance with the Medical
Treatment Act 1988 (Vic).

An administrator or person responsible in
accordance with the Guardianship and Administration Act 1986 (Vic),
such as the State Trust or any other person named as an
administrator in an administration order.

A parent, if the registrant is a
child.

A person otherwise empowered under law to
perform any functions or duties or exercise powers as an agent of or
in the best interests of the registrant.

A person who regularly acts on your
behalf with medical and care support provision.
Naturally we collect and hold a broad range
of personal, sensitive and health information gathered during the
course of providing our Safe Return Home service. However as
stated above, we strive to ensure that we collect and hold only
that personal, sensitive and health information which is relevant and necessary in providing
the Safe Return Home service to you.
2. Use and Disclosure
Purposes of Collection
We collect personal, sensitive and health
information primarily to assist with the speedy identification &
safe return of a person who has: been reported as

missing to Police, Emergency Services or a
Community Agency

appears lost and disoriented to a member of the public who is concerned for their
welfare

for your own information storage and distributional purposes
We will not use the information for any other
purpose.

have consented to us providing personal, sensitive and health

Sensitive and Health Information Data Quality
We aim to ensure that
your personal, sensitive and health information is accurate,
complete and up to date. To assist us in this, you or your
authorised representative needs to provide true, accurate, current
and complete information about yourself as requested and properly
update the information to us to keep it true, accurate, current and
complete.
If you or your authorised representative
believe that the information is inaccurate or incomplete, please
contact us and we will use all reasonable efforts to correct the
information. Alternatively, you or your authorised
representative may prefer to log on to our Safe Return Home
website, located at www.SafeReturnHome.com.au , and correct the
information yourself.
4. Data Security
and Retention
Securing Your
Information
We are committed to
maintaining the security and confidentiality of the data you or your
authorised representative has provided us and we will take
reasonable precautions to protect your personal information from
unauthorised disclosure, use or alteration. We have the
following security measures in place to protect your

Password Facility
Security to the system
is controlled by a secure password system whereby users have a login
and password assigned to them. The login and passwords are unique
for each person and they are instructed to keep them
confidential.
Site Encryption
To protect the username
and passwords, 128-bit SSL encryption technology is used to encrypt
access to personal, sensitive and health information on the website.
128-bit SSL is the same encryption used by commercial banks and is
sometimes referred to as "military strength". SSL ensures that all
data, including usernames and passwords, are encrypted during
transit from the user's browser to the website, and that pages
containing sensitive information are not "cached" on computers (i.e.
stored temporarily on computers via cookies). SSL encryption
is also used for any uploading or downloading of data directly to
the database.
Audit facilities
The database has audit
functionality so that a complete history of changes to information
is tracked by person (login) and time of access. The system also
tracks who is accessing the database.
Server Security
The server is stored in
secure and monitored facilities. Firewall and server configuration
follows security guidelines and industry best practices. Vendor
released security patches are applied regularly.
Retention of Your Information
All records of your
personal, sensitive or health information will be maintained by AACS
for the later of a period of 7 years after you or your authorised
representative last contact AACS or, in the case of a child, until
the child reaches 25 years. Where a record has been shown to
have been incorrect, that incorrect record will also be retained for
the later of a period of 7 years after you or your authorised
representative last contact AACS or, in the case of a child, until
the child reaches 25 years.

Openness
This privacy policy
shall be made available to anyone upon request. Further

6. Accessing
Your Information and Correction of Your Information
You or your authorised representative have a
right to access personal, sensitive and health information that we
hold about you. You or your authorised representative may
access the personal, sensitive and health information that we hold
about you at any time by logging on to our website located at www.SafeReturnHome.com.au. In limited
circumstances, we may refuse your request to access, in which case,
we will give you a reason for that refusal. We are not required to
provide access where:- denying access is required or authorised by
law; or providing access would be unlawful.
You or your authorised
representative also have a right to request correction of incorrect
information that AACS holds about you.

Identifiers
AACS does not assign
alpha or numeric identifiers to registrants.

Anonymity
Your identification is
crucial to the service provided by AACS in its Safe Return Home
program, therefore anonymity is not practicable.
9.
Transborder Data Flow
Due to the Australia
wide nature of the Safe Return Home program provided by AACS, it is
necessary to provide the registrant’s information to both Victorian
and interstate bodies. As part of registering for the Safe
Return Home program, you or your authorised representative have
consented to AACS providing your information to the following
bodies: Tasmanian, Victorian, Western Australian, New South Wales,
Australian Capital Territory, Queensland, South Australian, Northern
Territory, Police Department Officers
10.
Transfer or Closure of AACS
In the event that part
or all of AACS is sold, transferred or closed down, your information
will be handled in accordance with the statutory guidelines set out
in Health Privacy Principle 10.2 in the Health Records Act 2001
(Vic).

Making Information Available to another Health Service
Provider
If you or your
authorised representative request information held by AACS to be
transferred to another health service provider, we will provide it
as soon as practicable.
Changes to this
Statement
This Privacy Policy is
not a static document and we may make changes to it from time to
time for any reason. We will publish these changes on our
website.
